What is Bone-Marrow Aspirate Concentrate (BMAC)?

BMAC (Bone-Marrow Aspirate Concentrate) is the harvest of autologous bone-marrow containing mesenchymal stromal cells and other progenitor cells for regenerative applications. In Cyprus we use the Marrow Cellution™ system from Aspire Medical Innovation, a closed-tip cannula with graduated lateral side-ports that draws marrow from multiple medullary geographies in a single iliac-crest puncture.

The Marrow Cellution™ design produces concentrate directly in the cannula, with no separate centrifugation step. This shortens the procedure, removes a processing step, and avoids the hemolysis and cell loss associated with high-speed centrifugation. OrganoGEN is the exclusive Cyprus distributor.

The 2025 ESSKA-ORBIT European consensus on injectable orthobiologics (de Girolamo et al., Level I evidence) supports cell-based therapy as a second-line injectable option for mild-to-moderate knee osteoarthritis (Kellgren-Lawrence grade 1 to 3), with consistent pain and function improvement up to 12 months and longer-lasting benefit than corticosteroid or hyaluronic acid. In OrganoGEN's regenerative pathway, BMAC sits alongside PRGF (Endoret®), with PRGF as the first-line injectable and BMAC as the second-line option after first-line therapy or for indications where cell-based therapy is specifically preferred.

Am I a candidate?

Good candidates

  • Mild-to-moderate knee or hip osteoarthritis (Kellgren-Lawrence 1 to 3), as a second-line injectable after first-line PRGF or conventional therapy
  • Focal cartilage defects (knee, ankle, hip)
  • Bone non-unions and delayed unions
  • Tendon-repair augmentation
  • Bone-graft augmentation in combination with a scaffold (e.g. SmartBone® ORTHO)
  • Spine fusion adjunct (surgeon-selected)

Patients in whom another option may be preferred

  • Active local or systemic infection
  • Active malignancy at or near the target site
  • Severely compromised bone marrow or marrow-suppressive treatment
  • End-stage osteoarthritis (Kellgren-Lawrence 4) where joint replacement is the appropriate option
  • Severe coagulopathy or anticoagulation that cannot be safely paused
  • Patient-specific contraindications to autologous blood handling
